  • Residents can choose their doctor, manage medications with support, and participate in decisions about their health.
  • Meals and dining preferences are respected, with flexible options for location and timing.
  • The service prioritises safety, including falls prevention and dignity of risk, while promoting participation in daily decisions.
  • Residents are supported with appointments, health literacy, and ongoing monitoring to ensure care is safe, respectful, and responsive to changing needs.

Health professionals can refer prospective residents to Caladenia Nursing Home by contacting the Aged Care Assessment (ACA) to arrange an assessment. An ACA assessment, required by the Commonwealth Government, determines eligibility for residential aged care or respite.

Once completed, the prospective resident may tour the facility, submit an application, and discuss financial matters with our team.
