Aged Care Assessment


The Aged Care Assessment Program (ACA) helps older Australians and their families make informed decisions about care options. Whether you’re exploring support at home or considering short-term or long-term residential care, ACA provides expert assessments and guidance to match your needs. It forms part of the National Single Assessment System (SAS).

The Assessment team consists of Nurses, Social Workers, Physiotherapists, Occupational Therapists, Dietitians and Speech Pathologists.

Our team can visit you in your home, in residential care, or in hospital to carry out assessments—available Monday to Friday during standard business hours. It will take approximately 90 minutes.


What to expect from the service?

A qualified ACA Assessor who is a health professional — will visit you in the comfort of your home to talk through your care needs. You’re welcome to have a family member or carer present to support you during the conversation. We can also organise an Interpreter if English is your second language.

Together, we will explore your personal goals, what matters most to you, and the types of services or supports that can help you stay safe, independent, and well. The assessor’s role is to understand your situation and help tailor care options that suit your lifestyle and health needs. You can be approved for a variety of services and programs including;

  • Support at Home
  • Assistive Technology and Home Modification
  • Restorative Care Pathway
  • End of Life Pathway
  • Permanent Residential Care
  • Residential Respite Care
  • Transition Care Program
  • Commonwealth Home Support Program

Appointment information

What to bring:

  • A list of all the medicines that you take
  • All services that you currently use and the names of the people you call with their telephone number
  • If you’re on a Package your current budget and care plan
  • The name and contact details of your current General Practitioner
  • Medicare Card

Inclusion criteria:

  • An older person or Aboriginal or Torres Strait Islander 50 years and over
  • Residents living within the Vic 1 Aged Care Planning Region (ACPR) including Hume, Melbourne, Yarra, Nillumbik, Banyule, Whittlesea and Darebin Local Government Areas. ·
  • Homelessness Persons aged from 50 – 64 with no alternative pathway.

Exclusion criteria:

  • Medically unstable
  • Older persons that DO NOT consent
  • Younger Person NOT requiring Aged Care Accommodation Services.

Referrals:

ACA Community Assessment

The My Aged Care web referral will be screened and sent to the relevant ACA and the catchment they cover. Following receipt of referral; referrals are triaged within 24 – 48 hours. Triaging involves determining eligibility for assessment, priority status and ensuring referral contains all required information to proceed to an assessment.
