The Geriatric Evaluation Management at Residential Care program (GEM @ Resi ) provides care to people who are well enough to be cared for at an aged care facility but still require support from the NH GEM doctors, nurses and allied health.
Admission to GEM@Resi can be through an inpatient hospital service, an emergency department or from the community where a benefit can be identified.
Patients may be considered for GEM@Resi if:
- The patient is medically stable and ready for discharge from acute or subacute services
- The patient would benefit from time-limited care and support to complete their restorative process during a non- weightbearing period or whose long-term plan is permanent residential care.
- The patient would benefit from case management and low intensity physiotherapy.
- The patient is deemed ineligible for the TCP program.
- The patient does not require multidisciplinary support including Occupational Therapy, speech therapy, podiatry and dietetics.
The patient is in:
- Hospital (public or private) from an ED, SSU, Observation Unit, HITH or inpatient ward
- An ambulatory or continuing care serviceFrom the community where medically stable but at risk of an emergency department presentation
The patient (or their appointed Guardian) must agree (consent) to being supported by this program.
What to expect from the service?
- Safe and supported environment while unable to weight bear
- Returning to GEM/Rehab once weight bearing to complete Rehabilitation phase
- Long term care planning to residential care
- Stabilisation and management of medical conditions
- Access to nursing, physiotherapy and medical care
