The Primary Care Liaison team is the key point of contact for general practitioners (GPs) engaging with Northern Health. We work in close partnership with Primary Health Networks to deliver education and collaborative projects that support GPs in providing high-quality, coordinated care for their patients.
Our team offers advice and support to GPs and other community healthcare providers to strengthen communication, streamline clinical handover, and enhance care across the health system. We assist with navigating services, managing feedback, following up on hospital care, addressing clinical concerns, and facilitating referrals.
We also support the Shared Maternity Care model and deliver RACGP-accredited education activities tailored to the needs of primary care professionals.
