Northern Health is a prescribed Information Sharing Entity (ISE) under the Family Violence Protection Act (2008) and the Child Wellbeing and Safety Act (2005).
Northern Health has a procedure for Risk Assessment Entities (RAEs) and Information Sharing Entities (ISEs) to request and voluntarily share relevant information under the Family Violence Information Sharing Scheme (FVISS) and the Child Information Sharing Scheme (CISS) as outlined below:

Requesting or voluntarily sharing information with Northern Health
Requests to release information pertaining to the FVISS or CISS Schemes must be done by an authorised Risk Assessment Entity (RAE) or Information Sharing Entity (ISE). Northern Health will verify that the requesting organisation is an authorised service prior to sharing any information.
To request or share information with Northern Health about a patient:
- Complete: Family Violence & Child Information Sharing Form (Part A)
- Ensure that legal requirements and consent thresholds have been met.
- Email the completed form to:
General Hospital and Health Service
- Mental Health Division (for mental health information)
- Northern Health will review the request and provide a response within 3 business days.
